Note: There are two types of Screening Reports- ones requiring a waiver signature of the person being researched and ones not requiring a signature. On the reports requiring a signature, this is required by law for certain types of background checks.

In order to complete a screen where a signature is required, you must have the person being screened fill out the Screening Application. It is a simple one-page form that will ask the information we need to perform a search.
